I have had hundreds of guitars on the bench where the only thing "wrong" was that someone slapped a heavier or lighter set on and never touched anything else. Buzzing here, a note that won't intonate there, a wound string that pings out of tune every time you tune up. None of it is mysterious once you understand what a string set actually does to the instrument.
What a gauge change actually changes
A guitar setup is a balance between four things that all pull against each other: neck relief (the truss rod), string height at the nut, string height at the bridge, and intonation. String tension is the load that whole balance is set for. Change the tension and you nudge every one of those four. A full step up in gauge is roughly 20% more tension across the set, and that is more than enough to matter.
1. Neck relief and the truss rod
This is the big one. Your neck is a long piece of wood under tension, and the truss rod is the counter-force that keeps it from bowing forward. Put heavier strings on and you add pull; the neck bows a little more (more relief), which usually shows up as slightly higher action and sometimes buzz in the upper frets. Go lighter and you remove pull; the rod can now over-correct and pull the neck toward straight or even a slight back-bow, which gives you buzz down at the first few frets.
Here is the honest nuance techs argue about: not every neck reacts the same. Some are stiff enough to shrug off a gauge change and barely move. Others are lively and need a rod tweak the next day. That is why the smart move after any real gauge change is to string it, tune to pitch, and give it 24 hours to settle before you decide the relief is wrong. Wood moves on its own schedule.
2. The nut
Nut slots are cut to a specific string diameter. This is the part people forget, and it is the number one cause of "my new heavier strings won't stay in tune." A slot filed for a .046 low E is too narrow for a .052. The fatter string rides high in the slot and binds, so when you tune up it catches, then releases with a "ping" and jumps sharp. Same story on a wound third if you switch to a set with a plain G, or vice versa.
Going up a gauge means the slots may need to be opened slightly with the correct-size files. Going down is usually less dramatic, but a much lighter string can sit low and buzz in a slot that is now too wide, or occasionally pop out under a bend. Nut work is the one part of this I would not hand to a beginner with a hobby file — it is easy to go too deep, and you cannot un-file a slot.
3. Intonation
Intonation is where the string length is compensated at the saddle so it plays in tune up the neck. Different tension and mass change how the string behaves, so a gauge change will almost always throw your intonation off a touch. The good news: this is the easiest part to do yourself. Tune up, check the fretted 12th fret against the 12th-fret harmonic, and move the saddle until they match. Ten minutes with a tuner and a screwdriver.
4. Floating tremolos are a whole extra layer
If you have a Strat-style floating tremolo or a Floyd Rose, a gauge change is not optional maintenance — it is required. The bridge floats on a balance between string tension and the springs in the back. Heavier strings pull the bridge up and forward; lighter strings let it sag back into the body. Either way the guitar is now sitting wrong, the action shifts, and your relief moves with it. You have to rebalance the spring claw to match the new set, and on a Floyd you are also resetting the whole floating height. Hardtails and stopbar guitars are far more forgiving here.
So how big a jump actually needs a setup?
My rough rule from the bench: a small change within the same neighborhood — say 10-46 to a 10-52 hybrid set, or one brand's 9s to another's — you can often just restring and adjust intonation, then keep an eye on the relief for a day. A full gauge jump (9s to 11s, or 12s down to 10s), a switch that changes a wound string to plain, a move to or from a heavier bottom for drop tunings, or anything on a floating tremolo — that earns a proper setup. When in doubt, the tell is simple: put the strings on, and if it buzzes, won't intonate, or fights you at the nut, it is asking for one.
What you can do yourself vs. what to hand to a tech
You can absolutely handle intonation and, with care and the right hex key, small truss rod tweaks (a few degrees at a time — an eighth of a turn is a lot). The two jobs I would send to a bench are nut slotting for a heavier set and rebalancing a floating tremolo, because both are easy to overshoot and annoying to undo. There is no shame in it; a setup is cheap insurance against blaming the guitar for something that is really just a tension mismatch.

Will heavier strings damage my guitar?
No, not in any normal range. A modern electric neck and truss rod are built with plenty of margin for a jump from 9s to 11s. The neck just needs the relief rebalanced. The only real risk is ignoring a badly bound nut and forcing tuning, which stresses the string, not the guitar.
Can I go from 9s to 10s without a full setup?
Often yes. That is a modest tension bump. Restring, tune up, reset the intonation, and watch the relief for a day. If it stays comfortable and doesn't buzz, you are done. If the upper frets start buzzing, that is the truss rod asking for a small tweak.
Why does my guitar go sharp when I tune up after a string change?
